We respect clinical data integrity and legal privacy frameworks. All referral intakes, care coordination activities, and feedback loops are managed using SC Thrive’s Thrive Hub platform.

• HIPAA Compliant Workflows: Patient information is fully encrypted and protected under federal privacy guidelines.
• Closed-Loop Technology: Thrive Hub's closed-loop referral network ensures that patient tracking data, program attendance, and baseline biometric trends are automatically and securely reported back to your clinic's Electronic Health Record (EHR).

Please utilize the following clinical triggers and holistic risk thresholds when evaluating patients for HealthySoul recommendation:

• Target Diagnoses: Type 2 Diabetes, Cardiovascular Disease (CVD), Essential Hypertension, and/or Obesity.
• Biometric Thresholds: HbA1c ≥ 6.5%, Blood Pressure > 130/80 mmHg, or BMI ≥ 30.
• Social Health Drivers (SDOH): Patients experiencing compounding barriers to self-management (e.g., nutrition insecurity, financial strain, or non-emergency medical transportation barriers.)

To streamline order entries, please ensure your referral packet attaches standard clinical, demographic, and social risk codes to optimize patient risk-stratification:

• Clinical Diagnosis: ICD-10 codes including, E11.9 (Type 2 Diabetes), I10 (Hypertension), or E66.9 (Obesity).
• Social Drivers (SDOH): ICD-10 Z-codes including, Z59.41 (Food/nutrient scarcity) or Z59.81 (Transportation barriers).
• System Action: SNOMED CT 425228009 (Referral to community service).

Providers can confidently connect their patients to The GraceHub using three flexible, secure methods:

1. Secure Clinical Fax: Download our Standard Referral Cover Sheet and transmit securely to our clinical intake registry at (803) 632-8164. 
2. GraceHub In-Person Pickup: Email us a quick note, to share you have referral forms ready for in-person pick up and we'll stop by your office.
3. Direct EHR e-Referral: Select the "Referral to GraceHub / HealthySoul" order set within your EHR directory to transmit data via our secure integrated interface (scheduled live 2027).
