HealthySoul Whole Person Wellness is a comprehensive, community-engaged lifestyle medicine intervention designed to bridge clinical medicine with community. We provide individuals managing Type 2 Diabetes, Cardiovascular Disease (CVD), and Obesity with a supported pathway to lasting behavior change and clinical improvement with team-based, whole-person care.
Administratively housed within The Good Fruit Foundation, a South Carolina 501(c)(3) public charity, HealthySoul operates through the GraceHub—a centralized Community Care Hub (CCH). By combining clinical data integration, compassionate personal navigation, and local community resources, we ensure that a participants are supported in team-based, whole-person care.
We receive both self-referrals and provider-driven, key diagnostic (ICD-10) and social barrier (Z-codes) referrals for well-organized, connected systems from patient to provider to payer in a team based, person-centered care system.
Managed securely through the GraceHub, our dedicated Wellness Coaches and Navigators conduct person-centered assessments to resolve systemic barriers like nutrition insecurity or transportation gaps.
We connect participants with validated community-driven programs, including the Ardmore Institute's evidence-based Full Plate Living curriculum, and PCRM's Food for Life, in culinary medicine and nutrition education.
Utilizing SC Thrive’s Thrive Hub, our operational workflow is HIPAA-compliant. Thrive Hub safely tracks participant milestones, impact measures, securely routing "closed-loop" outcomes back to the primary care provider team.

To maintain the highest standards of clinical excellence, data security, and cultural relevance, HealthySoul is strictly evaluated and guided by two distinct oversight bodies at The Good Fruit Foundation,

Our clinical methodologies, community-based lifestyle interventions, and health plan alignment strategies are rigorously reviewed by our clinical board. This body is co-led by Dr. Neal Shealy, MD (Retired) and Sharon Shealy, FNP-retired, bringing decades of frontline rural and global medical expertise to ensure our pathways integrate seamlessly with mission-based healthcare systems.

True whole-person wellness honors the emotional, mental, and spiritual fabric of the communities we serve. Our programmatic outreach, cultural adaptability, and volunteer networks are guided by our Spiritual Advisory Board, led by Miho Yasukawa of the Fairfax Lutheran Parish. This board helps ensure our interventions remain deeply rooted in compassion, equity, and local trust, serving faithfully the love of Christ in action.

Beloved, I wish above all things that thou mayest prosper and be in health, even as thy soul prospereth. | 3 John 1:2
